  • Kindly provide 24 hours notice to cancel any appointment.

  • We reserve the right to charge for appointments not kept or canceled without 24 hours advance notice 

  • Please inform us If there has been any change in your medical history or contact information since your last visit.

  • Payment is due at time of service.

  • Payments can be made by check or cash.

We recognize the global health pandemic taking place in the world right now and have outlined the following policies for patients and clients during COVID-19.

  • 491 Coolidge Highway is considered a medical building so everyone must wear masks regardless of vaccination status.

  • Masks can be removed during treatment.

  • I am fully vaccinated with Moderna.

  • I generally get a rapid test once a week.

  • This office uses a portable HEPA Air Purifier.

  • This office follows the most up-to-date methods of sanitation and sterilization.
